I understand that workshops offered by Mindfully Well PLLC are informational, educational, and wellness-based, not psychotherapy or medical treatment. Participation is voluntary, separate from any therapy services, and will not affect my therapeutic relationship. Fees are independent from therapy charges and are not billable to insurance. I acknowledge that I should always consult with my medical provider for personal health concerns.
